When it comes to hot sauce which I love, I’m always on the look-out forhot sauces that have a great deal of flavor beyond the heat…and a non-thermonuclear dose of heat, at that. What good does all that fire do, I always wonder, other than prove your intestinal fortitude? So I was thrilled to recently discover a Portuguese product that does exactly what I want hot sauce to do.
In Portugal, that wonderful but oft-overlooked food country, there is a ubiquitous sauce called molho de piri-piri or, more commonly, piri-piri. It is based on a type of red chili (the piri-piri) that once upon a time made the journey from Brazil to Portuguese Angola, and now, growing profusely in Angola, is brought to Portugal by the ton.
I’ve had the sauce all kinds of ways: dried peppers steeped in oil, fresh peppers blended with oil and vinegar to make a thicker sauce. This particular piri-piri, bottled in Portugal, is of the latter type—and the best piri-piri I’ve ever tasted in the U.S. It is a bright orange-ish red, about the thickness of a thin tomato juice (much thicker than Tabasco, for example). It has lots of specific chili flavor, as well as a nice, vinegary acidity—but is only moderate in heat, so you can really use this stuff to add flavor, not just BTUs.
In Portugal, it is de rigueur on grilled chicken. Here’s an American translation: if you use it as your hot sauce in preparing Buffalo Chicken Wings (in the classic upstate New York recipe, you mix hot sauce with melted butter), your wings will really take off.
The Quinta d’Avo Piri-Piri is available through The Spanish Table, which has three great stores on the West Coast. Shipping is extra.
Dean and DeLuca Cookbook
American food is no longer just mashed potatoes and roast beef. For over two hundred years, American dinner tables have welcomed the ethnic, regional, and home-style fare of many different places and people. Some of these dishes -- Ravioli, Beef Tacos, Swedish Meatballs, Beef Stroganoff, Potato Salad -- have been around so long they seem as American as anything else. We've also greedily appropriated all kinds of regional specialties, dishes like New England Clam Chowder, Gumbo, Cornbread, and Texas Barbecued Brisket. And then there are the home classics that seem to come from everybody's grandmother's kitchen. As David Rosengarten makes deliciously clear in It's All American Food, these foods have become American, and together they form an exciting and unique American cuisine.
The author of the bestselling Dean and DeLuca Cookbook, David brings together all the wonderfully diverse foods that we love to eat: the best ethnic dishes, the best regional dishes, and the best classic American home cooking. Only here will you find the most authentic Cajun recipes side by side with thrilling Chinese-American dishes you can cook at home, the timeless Brooklyn-Italian meals alongside the sizzling Cuban specialties of Miami… not to mention that essential centerpiece of mashed potatoes and pot roast you once looked forward to every Sunday.
The endocrine system encompasses several glands in different parts of the body. These glands secrete directly into the bloodstream and affect various important functions. Hormones secreted by the endocrine glands work as chemical messengers of the body. The delicate balance of endocrine secretions can be disturbed by stress, infection as well as other factors. Be informed of a few endocrine diseases such as those of the thyroid, pituitary or pancreas. Take a look at some of the effects of endocrine system disorder.
Endocrine system
The endocrine system plays a crucial role in important functions such as digestion, reproduction and homeostasis (constant internal balance). The endocrine system is primarily made up of the hypothalamus, pituitary, thyroid, parathyroids, adrenals, pineal body and the reproductive glands. The endocrine secretions work along with the immune system and the nervous system to aid the body in various situations. Endocrine glands secrete major hormones directly into the bloodstream. This is then carried to all parts of the body.
The hypothalamus is the hub for the endocrine and nervous systems. It controls the functioning of the pituitary gland.
The pituitary secretions exercise control over other endocrine glands. Some of the important hormones secreted by the pituitary are the growth hormone, prolactin, corticotropin, endorphin and thyrotropin.
The thyroid hormones are vial to the development of brain and nervous system in children.
Endocrine disorder
Endocrine system disorders occur either due to too much or too little or sometimes no hormone. These disorders may lead to abnormal growth pattern, osteoporosis, diabetes, high cholesterol and triglyceride levels and thyroid malfunction. Endocrine system disorders include hyperthyroidism, hypercalcemia, growth hormone deficiency, Addison’s disease, Cushing’s Syndrome and hypothyroidism. Endocrine disorders can be triggered by tumors, steroid medications or autoimmune problems. A person suffering from a disorder caused by any of the malfunctioning endocrine glands may notice change in weight, change in libido, mood swings, increased fatigue and thirst or urination. Endocrine disorders are caused due to malfunctioning endocrine glands. In some cases, the gland produces in excess while in others there is insufficient activity of the gland. Inadequate secretion of endocrine glands (hypofunction) can be attributed to malformations or disease or injury. Excessive production of hormones (hyperfunction) is usually caused by glandular tumors or auto immune reactions. Endocrine disorders are treated with hormone replacement in cases of inadequate production. In cases of excessive production of hormones, the abnormal gland tissues are removed.
Endocrine system disorder Growth Hormone Deficiency – When a child suffers from inadequate amount of growth hormones, the child may have an immature face and chubby build. The rate of growth of all body parts is slow. Growth hormone deficiency can be total or partial. This endocrine disorder can be diagnosed with blood tests that measure the hormone concentrations and x-rays of hand and wrist bones to determine the bone development. Growth hormone deficiency is treated with growth hormone injections. Treatment usually goes on for a few years until acceptable growth is achieved.
Hypopituitarism - This endocrine disorder is sometimes congenital occurring due to abnormalities in the formation of the pituitary or hypothalamus. It can be caused by a brain tumor or infections of the brain and its surrounding tissues.
Hypercalcemia – This endocrine disease is caused when the level of calcium in the blood is too high. Calcium level in the body is maintained by Vitamin D and Parathyroid hormone. Hypercalcemia is characterized by bone pain, nausea, kidney stones and hypertension. A person may notice bone pain and spinal chord curvature. Other symptoms are irritability, muscle atrophy and loss of appetite.
Addison’s disease – This hormonal disorder is caused by inadequate production of cortisol by the adrenal glands. Symptoms of Addison’s disease include weight loss, loss of appetite and fatigue. Addison’s disease is also known as hypocortisolism. An important fallout of this endocrine disease is hyperpigmentation – darkening on several parts of the body. The deficiency of cortisol can lead to irritability and salt craving.
Cushing's syndrome - This endocrine disease is caused due to excessive production of cortisol and is also referred to as ‘hypercortisolism’. Typically persons suffering from Cushing’s syndrome experience upper body obesity as also severe fatigue, weak muscles and fragile bones. This endocrine disorder is the opposite of Addison’s disease.
Acromegaly – This endocrine system disorder is a result of excessive production of growth hormone. This disease may be difficult to recognize and diagnose as it sets very slowly in middle aged adults. It is characterized by enlarged hands and feet. This abnormal growth is also noticed in the facial features such as the lower jaw, nose bone and the brow. Persons suffering from this endocrine disease may experience enlarged liver, spleen and kidneys. Diabetes, hypertension and cardiac ailments are common fallout of this endocrine system disorder.
Hashimoto’s disease – This is a case of chronic thyroiditis that is caused due to a reaction by the immune system against the thyroid gland. This disease is hereditary and is manifested with symptoms like mild weight gain, intolerance to cold, dry skin and hair loss. A woman may notice heavy or irregular menstrual cycle.
Hypoparathyroidism – This disease of the parathyroid glands is caused due to insufficient levels of calcium in the blood due to the inadequate levels of parathyroid hormone. It can leave a person with tingling feeling in the hands and muscle cramps. Typically this endocrine disease takes years to manifest completely.
Hysterectomy
Hysterectomy is probably one of the most common surgical procedures performed on women. It involves removal of the uterus and often the ovaries and fallopian tubes too. A hysterectomy can be painful, physically and emotionally to a woman. It can bring about a sense of loss of femininity. But often it can give great relief from symptoms that were affecting the quality of life of a woman. A physician would elect to do an abdominal hysterectomy or vaginal hysterectomy depending on the individual conditions. Laparoscopic hysterectomy reduces the time of recovery from hysterectomy surgery.
Hysterectomy
Hysterectomy is the surgical removal of the uterus that can sometimes save the life of a woman. Hysterectomies were usually performed abdominally or vaginally. Laparoscopic hysterectomy is also performed in some cases. Examine some of the conditions that lead to a decision of hysterectomy surgery:
Gynecological cancer – Cancer of the uterus or cervix usually necessitates removal of the uterus and cervix. The surgery is decided based on the kind of cancer and the extent of advancement of the disease. Endometrial cancer, cancer of the uterus or cervix or fallopian tubes usually requires hysterectomy surgery.
Endometriosis – In some cases of severe bleeding following endometriosis, a hysterectomy surgery is advised. This is the second leading reason for hysterectomies.
Fibroids – While most fibroid tumors can be treated by non-surgical methods, hysterectomy surgery may be the only permanent solution.
Uterine prolapse – When the uterus moves from its usual place down into the vagina, it can affect other organs such as the bladder. This can happen due to childbirth, obesity, loss of estrogen after menopause and weak pelvic ligaments and tissues.
Other reasons for going for a hysterectomy surgery range from heavy or irregular menstrual periods to dysmenorrhoea. Severe pelvic inflammatory disease can sometimes necessitate a hysterectomy.
Types of hysterectomy surgery
Total hysterectomy involves removal of the cervix and uterus. It is known as Oopherectomy.
Partial hysterectomy is a surgical procedure to remove only the upper part of the uterus, leaving the cervix in place. This type of hysterectomy is referred to as supra cervical hysterectomy or subtotal hysterectomy.
Radical hysterectomy involves removal of the uterus, cervix, upper part of the vagina and the supporting tissues.
Hysterectomy surgery performed prior to menopause can bring about severe and prolonged symptoms as compared to natural menopause. A woman can experience bone loss, loss of libido and emotional difficulties too. Depression and anxiety are not uncommon. Some women fear that their sexual life will be affected due to the hysterectomy surgery. In cases where the length of the vagina is shortened, a woman may experience pain during sex. Many women may need hormone replacement therapy (HRT) following total hysterectomy where the ovaries are removed before menopause.
Hysterectomy vaginal
During a vaginal hysterectomy, an incision is made in the upper portion of the vagina and the uterus is removed through it. This is done in cases where the uterus is not too large as well as uterine prolapse. With vaginal hysterectomy, there won’t be any external scarring. Recovery is also faster when compared with abdominal hysterectomy. This type of hysterectomy surgery can be done with laparoscopic assistance. The laparoscope is inserted into a small incision in the lower abdomen and this allows the surgeon to examine the pelvic organs. Specially crafted surgical instruments can help in detaching the uterus and removing it vaginally.
Hysterectomy recovery
Since a hysterectomy is performed under anesthesia, it could take a few hours for you to recover from the surgery and probably a few weeks to get back to your usual self. You will be observed for any signs of pain and discomfort. Some complications that can arise after hysterectomy surgery are blood clots, excessive bleeding and infection. Recovery from abdominal hysterectomy takes 4 – 8 weeks. It is essential to take plenty of rest and begin with light chores initially. Fever is noticed in some women post hysterectomy surgery. Rarely there is damage to the abdominal structure or organs. Women who have undergone vaginal hysterectomy may experience and inability to pass urine.
Following a hysterectomy, there is an increased risk of vaginal vault prolapse. This is a condition where the top of the vagina drops down due to a reduction in support structures. You can recover from a vaginal hysterectomy or laparoscopic hysterectomy faster, probably within a couple of weeks. You may return to work as soon as you have sufficient stamina and mobility.
Try and get up and walking in a day to keep blood circulating and prevent blood clots from forming. Blood clots can occur in the large veins of the leg or lung.
Avoid lifting heavy objects for about 6 weeks after the hysterectomy surgery.
Eat a balanced diet with iron replacement to promote healing.
Avoid constipation by drinking plenty of fluids
Follow the prescribed exercise program
Call your physician should you experience increase in pain, persistent nausea or vomiting, heavy bleeding or signs of infection.
Abdominal hysterectomy
Abdominal hysterectomy is preferred when the uterus is enlarged or there are large fibroids, extensive adhesions or endometriosis. In cases of cancer of the uterus, ovaries or cervix, an abdominal hysterectomy is undertaken. Abdominal hysterectomy can be done with a vertical or bikini line incision. This incision is typically 10 – 15 cms long. The uterus and cervix is removed by cutting it off at the top of the vagina. The top of the vagina is repaired by sewing. This is called the vaginal cuff. This type of hysterectomy surgery reduces the incidence of damage to the urinary tract. But it involves a longer hospital stay and recovery time.
Hysterectomy laparoscopic
Laparoscopic hysterectomy surgery involves making three or four small incisions in the abdomen. This allows the laparoscope to enter the abdomen or pelvis. The uterus is cut up into small pieces and removed through the tubes that are inserted into the abdomen. It involves shorter hospitalization and recovery time. But it can be a costly surgical procedure when compared to traditional abdominal or vaginal hysterectomy.
Dry Eye Syndrome
Dry eye syndrome is a condition that is characterized by chronic lack of sufficient lubrication and moisture in the eye. Nearly 10 million Americans suffer from dry eye syndrome. While many suffer from this condition as a result of the normal aging process, there are other causes that contribute to it. Read about the causes of dry eyes and what you can do to tackle this condition. Treating dry eye syndrome is not only about relief from discomfort but also protecting the cornea.
Dry Eye Syndrome
Tears perform the task of keeping the eyes well lubricated. Tears bathe the eyes, wash out dust and debris and keep the eyes moist. They also contain enzymes that neutralize the microorganisms that are found in the eyes. Tears keep your eyes moist, help ocular wounds heal and guard against infection. They also wash away allergens and protect the eye. The quality of the tear film lubricating the eye is responsible for dry eyes. In dry eye syndrome, the eye doesn't produce enough tears or the tears have a chemical composition that causes them to evaporate quickly. Dry eye syndrome is common fallout of increasing age since the body produces less oil over time. The oil deficiency has a bearing on the tear film since the water layer over the eye dries faster. This condition is more pronounced in women than men. Women suffering from dry skin or menopausal symptoms may notice dry eyes too. There can be many other causes for dry eyes:
Hot, dry and windy climate
High altitudes
Cigarette smoke
Side effect to medications such as antihistamines, antidepressants, certain blood pressure medicines, Parkinson's medications and birth control pills
Insufficient blinking such as when you are reading or working on the computer
Incomplete closure of the eyelids, eyelid disease or deficiency of the tear-producing glands
Contact lens wearers may also suffer from dry eyes because the lens absorbs the tear film and causes proteins to form on the surface of the lens. Contact lens intolerance can also be a symptom of dry eye.
A person suffering from dry eye syndrome feels a persistent itchy and burning feeling. There is a ‘foreign body’ sensation within the eye. The patient may feel persistent dryness and scratching in the eyes. Surprisingly watery eyes may also be a symptom of dry eye syndrome since the tear glands may be over stimulated into producing tears that do not help in lubricating the eye. Tears of this kind as well as those produced as a result of reflex response to outside stimulus or emotional reaction do little to soothe dry eyes. A person suffering from dry eyes is likely to feel discomfort after extended periods of computer or TV viewing. A significant percentage of patients also experience photophobia and intermittent blurring or other visual problems. Stringy mucus may appear in or around the eyes. Even with less severe symptoms, persons suffering from dry eye experience reduced work capacity and decreased quality of life.
Treating Dry Eye Syndrome
A condition of dry eyes is determined by a physician who will measure the production, evaporation rate and quality of tear film. A thin strip of filter paper placed at the edge of the eye, called a Schirmer test, is one way of measuring this. An ophthalmologist can look at the film of tears on your eye using a slit lamp (bio-microscope). A dye such as fluorescein may be placed in the eye to make the tear film more visible.
Treatment for dry eyes involves trying to preserve as much of the eye’s natural moisture as possible. Eye drops act as artificial tears and provide temporary relief. Anti-inflammatory agents are prescribed for more severe cases of dry eyes. While topical steroids can be used for inflammation, they can cause side effects after prolonged use. Some forms of dry eye syndrome benefit from the placement of tiny plugs in the ducts that drain tears from the eye. These special plugs trap the tears on the eye, keeping it moist. This may be done on a temporary basis with a dissolvable collagen plug or permanently with a silicone plug. For severe cases, special goggles called moisture-chamber spectacles can be worn.
Make a conscious effort to blink regularly so as to spread the tears more evenly.
Maintain indoor humidity of about 30 – 50%.
Keep the body well hydrated with plenty of fluids and water.
Follow good eye-care practices. Do not ignore burning sensations in the eyes or persistent and painful grittiness. Avoid rubbing the eyes.
Wear sunglasses with maximum ultraviolet protection.
Wear proper eye safety equipment when necessary.
If you suffer from dry eyes, inform your health care provider when he prescribes medications for other ills.

Gmail's new feature
Let’s go through them, shall we?
01. Reply on top: Now we don’t have to scroll all the way to the bottom of a message to click “Reply.” How convenient! As if we weren’t already lazy enough…
02. Embarassment-reducing new message notifications: Thank you thank you! This will keep me from looking the fool when someone else in the loop knows better. Again thank you! Actually, those in my mailing groups should thank you because they will be notified as soon as I send my reply. Then they won’t have to waste their time with a response.
03. Forward all: You can now forward the entire conversation. I’m not sure how useful this will be since most of the replies will copy the previous message anyway, but WOOHOO. I can forward all!
04. Chat even when your friends are offline: Gmail will now serve as my friends’ answering machines! SUPER because I was too busy to just email them anyway…
05. Get Gmail on your mobile phone: Yes, but is it free? Will it use up my minutes? Yeah, didn’t think so.
Thank you, Google, for all these fun features. I’m sure someone out there will find them even more useful than I have. (Don’t get me wrong - I heart Gmail… I’m having issues, and the little exclamation at the top of my email was asking for it. I never had the bitch factor in me, just the whiny factor.)
